Welcome to a new school year at Summit School! Below you will find information on our after school program for the 2012-2013 year. Please read over this packet carefully as it contains information regarding this program and requires a signature for your child to attend. Even if you are a returning family and have signed a contract in the past, a new one must be signed each school year in order for your child to participate in Condor Kids Club. The attached form must be signed and filled out and submitted to the PTO box in the school office as soon as possible, preferably by the first day that your child will be in the program.

Kindergartners: M,
T, R, F : 1:00-2:45* then 2:45 to 5:00 with all grades
Wed:
12:15-5:00pm (early dismissal day)
*At 2:45 all grades Condor Kids Club Begins and runs until
5:00pm
Parents of Kindergartners: Note that your child should be
picked up from school each day by 1:00, except Wednesday when school is out at
12:15. If your child is not picked up by 1:00 on M, T, R, or F or by 12:15 on
Wednesday, he/she will automatically be signed into after school care.
Condor Kids Club: M, T, R, F: 2:45 – 5:00pm

Summit School may participate in Condor Kids Club.
Bills are sent via email
monthly. Please make sure you include your email address on the attached form.
If you do not use email, please contact me. Bills are due upon receipt.
Condor Kids Club is run by and benefits the PTO of Summit
School. The PTO has approved all persons who oversee the program; fingerprint
cleared through Ojai Unified School District, and will be enforcing all school
rules during the program. Our goal is to provide a safe, fun after school
program for all of the families at Summit. If you have any questions or

Condor Kids Club: General Information
Our after school program is likely to include Free play
time, both indoor and outdoor, opportunity for quiet time for homework,
organized activities and play and a nutritious snack and drink.
All children will be expected to follow all school rules
during Condor Kids Club hours.
Billing/Payments: Bills will be sent via email each month.
Payment is due upon receipt and considered late 15 days following date of
invoice. If a bill remains unpaid for 90 days and/or a balance of more than
$200.00 is carried over we will ask you to make other after school arrangements
for your child until the bill is paid in full. Balances over 120 days past due
will be sent to collections.
Check should be made payable to : Summit School PTO, on the memo line please write: Condor Kids
Your child is welcome to stay on a regular basis or just
occasionally. Please make sure your child knows what the plan is for the day.
If your child needs to stay in Condor Kids unexpectedly please call the school
cell phone at: 805-701-3198 and let us know. The adult in charge of Condor Kids
will have that cell phone on them during all Condor Kids hours. If a child is
not picked up when the school day is over, and school staff is not familiar
with family plans for them to walk/ride bike home, they will signed automatically
into Condor Kids Club. This is for safety reasons.

RESPONSIBILITIES
To help us provide a safe and fun environment for your
child, we ask that you observe the following:
- Your child must be picked up your by 5:00pm. There are no exceptions. Additional fees will result from late pick-ups. We realize emergencies happen, and pick up may be delayed. Please call the cell phone: 805-701-3198 if you have such an emergency.
- We need to know who will be picking up your child each day. If someone other than a parent will be picking them up, please make sure they are listed on the Schools emergency contact form and you notify Condor Kids that day. We will not release your child to anyone else.
- If your child will be going home in a manner other than ordinary, for example riding their bike home or going home with a friend, we need to be notified by a parent/guardian. No exceptions. The child will not be permitted to leave until a parent/guardian can be reached. This is for your child’s safety.
- All adults picking up a child must sign them out. So you must get out of your vehicle and come in to check your child out, and sign them out on our log in/out sheet and make contact with the staff member.
- Please notify us of any changes to your emergency contacts form.
- If there is any person that your child may NOT leave school with, such as a non-custodial parent, please notify Kimberly Rivers directly. All information is considered private and confidential.
- Please make sure all allergies and/or medical conditions or any other special instructions are on your emergency card AND that you notify Condor Kids as well.
- Please notify us directly of any dietary restrictions/preferences.
- We will provide a quiet time and place for your child to work on homework, however we are not responsible for the completion or correction of homework. The work should be reviewed at home each day.
- If you have any problems with any employees, or questions of any kind please feel free to contact the director, PTO President and/or the school principal at any time. For serious issues we ask you to fill out a complaint form, available in the office and appropriate measures will be taken immediately. The safety and welfare of the children is our first and foremost priority.
